The RN Skilled Unit Manager is responsible for ensuring the delivery of exceptional resident care of all skilled nursing residents. This role focuses on the education of nursing staff, oversight of the admission process, and ensuring the accuracy and compliance of care plans and documentation. The manager reports to the Director of Nursing and collaborates closely with interdisciplinary teams to enhance resident outcomes and ensure seamless operations.
ESSENTIAL JOB FUNCTIONS
Works collaboratively with the Nursing staff and team members in other levels of care to coordinate admissions, discharges and transfers, determining levels of care and ensuring smooth transitions from one level to the next
Reviews admissions documentation for accuracy and completeness to include medication reconciliation, verification of orders, and opportunities for staff training and process improvement; participates in weekly Medicare meetings to discuss residents rehab status and discharge plans
Coordinates with Admission Coordinator to streamline resident admission process and verify all required pre-admission documents are secured prior to resident arrival to community.
Oversee the admission process upon resident arrival to ensure all clinical requirements and the admission checklist have been completed by assigned staff per regulations and Williamsburg Landing job routines, policies and procedures.
Attends clinical meetings, resident care plan meetings; attends IDT, Risk Management, COR and QAPI Committee meetings; provides reports on assigned quality metrics
Verify residents’ appointments during stay have been scheduled and follow up recommendations have been communicated with the resident’s responsible party and Williamsburg Landing reception and transportation as applicable.
Communicates and follows up on pharmacy recommendations and lab and diagnostic results with appropriate medical provider.
Ensures the integrity of infection control policies under the direction of the Infection Prevention Nurse.
Assigns resident care responsibilities to Nursing staff and ensures care plans are developed, followed, and revised as needed
Assists the Director of Nursing in verifying competency with regard to medication pass, reviewing physician orders for accuracy and completion Routinely monitors skilled nurses’ notes and aide’s ADL documentation to ensure accuracy and compliance with regulation
Rounds daily, ensuring skilled nursing services are performed in accordance with acceptable nursing standards and takes immediate and appropriate action to correct deficiencies
Educates and mentors nursing staff on clinical tasks and best practices; provides ongoing training to ensure adherence to clinical protocols and standards
Collaborates with the Social Services/Discharge Planner to ensure resident readiness for discharge including resident education and medication reconciliation
Tracks changes in resident condition and ensures adjustment to the plan of care
Manages Skilled Unit resident and/or family concerns and reports to the Director of Nursing and Social Services/Discharge Planner for follow up
Works as a staff nurse when necessary and assumes on-call responsibility as assigned
Performs all other duties as assigned
QUALIFICATIONS
Two or more (2+) years experience in skilled nursing or rehab facility, required
One (1) year of experience in Nursing Supervision, preferred
Knowledge of state and federal Long-Term Care regulations, preferred
Current Commonwealth of Virginia Registered Nurse license, required
Experience with MDS tools, and assessment and planning processes, preferred; Current MDS Certification or interest and ability to obtain MDS Certification within 12 months, required
KNOWLEDGE, SKILLS AND ABILITIES
Comprehensive knowledge of geriatric and skilled nursing care standards, principles, and practices and of state and federal regulations related to nursing homes
Demonstrated experience with electronic medical records systems, and knowledge of clinical workflows, data management, and nursing documentation standards
Knowledge of Medicare and Medicare Advantage guidelines
Ability to work cross functionally and collaboratively with staff and other operational and clinical leaders to ensure resident and family satisfaction during admission, stay and discharge
Excellent organizational and administrative skills and the ability to handle multiple priorities simultaneously in a fast-paced environment
Ability to assist licensed personnel as necessary
Ability to regularly and safely lift a minimum of 35 lbs.
Ability to use a variety of health care equipment such as lifts, thermometer, and blood pressure cuff
